If you are planning to compete at the Grant Leavitt Skate Jam tomorrow, you will need to complete a Release and Waiver of Liability and Indemnity Agreement. We will have them available at the event tomorrow. But if you want to get a jump start on it, you can download it now and bring the completed form with you.
IMPORTANT- IF YOU ARE UNDER 18 YEARS OF AGE, A PARENT OR LEGAL GUARDIAN WILL NEED TO COMPLETE THIS FORM.
